LRID- is an Approved RID CMP Sponsor for Continuing Education Activities.

This form should be completed by the activity's contact person and/or the presenter/instructor of the activity. This form needs to be completed at least 45 days before the activity. A check made out to LRID needs to be mailed to the CMP Administrator using the following chart to determine the amount.
